The Medical Certificate is a 1-2 day medical or carers certificate for yourself or someone under your care. You will be required to complete a short questionnaire when booking plus a brief phone consultation. The medical certificate will be emailed to you within minutes of the practitioner reviewing your questionnaire responses.
For medical certificates required for more than 2 days, patients must consult with a doctor through an Our Sage Telehealth Consultation.
Please note Our Sage does not issue medical certificates for third parties including for court appearances and parole compliance.
